PHONE CONSULT
New Clients are required to call or text to schedule a complimentary phone consultation. A brief health history will be taken. Appointment may be scheduled at this time.
INNAPROPRIATE BEHAVIOR WILL NOT BE TOLERATED
The client understands that this is a professional massage by a Massage Therapist licensed by the Ohio State Medical Board. The client understands that any sexual advances/remarks or behaviors will result in an immediate termination of the session. You will be reported to authorities.
WHAT TO WEAR
Sessions are done FULLY DRAPED with sheets and a blanket. Clients may dress within their level of comfort (fully clothed, partially clothed, or down to undergarments and draped).
ARRIVE EARLY
Please arrive 10-15 minutes early to your first appointment to discuss intake form/treatment plan with your therapist. Thereafter, please arrive 5-10 minutes prior to your scheduled appointment
LATE/CANCELLATION/NO SHOW
A reminder will be sent at least 24 hours before your scheduled appointment time. If you will be late, please let us know as soon as possible. If you will be more than 10-15 minutes late, you may need to reschedule. Cancellations with less than 24 hour notice may be subject to a $25.00 fee. No call/No shows may be charged the full session price.
ILLNESS
You must reschedule your appointment with at least a 24 hour notice if you are not feeling well, have a fever, cough, chills etc. Please give us time to try to fill your appointment and let us know as soon as possible. Integrity Wellness will refuse service to anyone with a contagious illness.
MINORS
Children (under the age of 18) that are receiving a massage need to have an adult on premises during the entire time of the appointment. The parent/guardian may stay in the room during the whole time of the session at any age if preferred.
PAYMENT
Payment for services can only be processed by check or cash until new payment processing is set up. We apologize for any inconvenience.
